leave system form request?

mrDj. can i make a request?
about this leave system.
can SULYAPINOY, HElp provide us a "leave FORM" in korean with english subtitles.

so that it would be easy explaining to our employer IF we applied for a leave.
much more formal rather than giving the employer a copy of the labor code.

this form would help a lot. THANKS in advance.

i understand and agree on your point...

actually, there is no standard form for "vacation leave"... it will depend on each company on how they process their leave application and approval... just like in my company, we apply our vacation leave thru our company website and there is no need to fill-up using a form... i know most companies use a form but i am sure that the form design/content is different...

maybe as a reference, para hindi na kayo mahirapan mag-explain if you apply for a vacation leave, i will upload a "sample leave form" soon... but please give me enough time to get a sample copy of leave form...

for other sulyap users, if you have a sample leave form there, i would appreciate if you could scan it and email to me at sulyap.managing@gmail.com. i will try to edit and add some english translation on it...
