1. Original Passport

2. Colored "SCANNED" Passport [1st page only]. Make sure the copy is very clear

3. 2 pieces 2X2 pictures [hindi po passport size] with name tag & red background

4. Medical result [original & xeroxcopy]. pirmahan po ang medical result

... isang kopya lang po ng passport [scanned] pati rin po ang medical result. at saka mahalaga rin po na i-update ang account sa e-Reg base po sa datos ng medical result [height, weight etc.]
